HMO plans
PBC works with select HMO plans connected to Advocate, Silver Cross, and Little Company of Mary medical groups. HMO coverage generally requires the correct medical group assignment and any required referral or authorization.
Before scheduling
Insurance participation can differ by clinician and specific plan. Please provide complete insurance information during registration so PBC can verify eligibility, benefits, copays, deductibles, and authorization requirements. Verification is not a guarantee of payment; final coverage is determined by your insurance company.
PBC may also provide services on an out-of-network or self-pay basis. PBC does not currently participate with Medicare.
